This just drives me crazy! I realize certain words are used differently when dictating/transcribing medical reports; however, I am accustom to saying, "Instructions were given in writing." My doctor says, "Instructions were given in written." Is this phrase common among physicians (in written)?

Well, I've certainly never heard it in over 40 years. If you're allowed to change it, I'd go with "in writing" or "Written instructions were given".

Never heard of that one either. I do have a doc who says "in lieu of" when he means "in light of." He says it when he really means in lieu of also, but almost 99% of the time he's using it "in lieu of" in light of!

I even looked it up when he first said it because I thought I had missed something!

Just can't resist throwing in my 2 cents' worth here, even though I'm - what - 3 weeks? - late? the doc was probably saying "given AND written", meaning: They were given (verbally) and also written.
