I just cannot seem to find this. I was wondering if anyone could help me with this one:
The patient had been seen multiple times in the outpatient clinic with a diagnosis of s/l*type 4 or type 0* hyperlipidemia and a history of acute pancreatitis with two episodes in January and February.
1. s/l*Type 4 or type 0* hyperlipidemia with elevated triglycerides and cholesterol.
2. Acute pancreatitis with probable ascites. Rule out pseudocyst or supra-imposed candidiasis. The fullness of the cul-de-sac may be secondary to ascites, s/l???????? of gynecologic problem.
(On this one the accent is terrible and I cannot really even tell what it sounds like. Something like lockidet, rockidet??
I don;t know. I am totally lost on this one.)
