a moist Ray-Tec was removed that had been placed. (is it Ray-Tec or Raytex)?

At this time the mandibular incision was closed in the following fashion, copiously irrigated with irrigant and then closed using running 3-0 chromic in a running horizontal interlocking metris fashion bilaterally. (is metris correct?)

Good morning,

I have a questions on Quiz 37 Part 2. Karen Dubose

1. Is it safe to assume I can use format 1 for the discharge summary?

2. Under Physical examination on admission: are we supposed leave the report the way they typed it? or are we supposed to put it into format? For example: Blood pressure 94/50, pulse 160 . . . vs. Blood pressure: 94/50. Pulse: 160. Respirations: 52. (and so on)

3. Lab on admission: 3rd line - CK 1068 and 4th line - 9000 - they did not put a comma, (I did) either way is correct? Yes?


If I fix the formatting errors, physical examination on admission it should read:

PHYSICAL EXAMINATION ON ADMISSION
Blood pressure: 94/50. Pulse: 160. Respirations: 52, and labored breathing was noted. Temperature: 100.8 F. Numerous petechiae on the lower extremity were noted, and the patient was in severe respiratory distress. HEENT: Showed no trauma, and the pupils were normal. Respiratory exam showed bilateral rhonchi. Chest: Heart: Exam showed elevated venous pressure in the neck, tachycardia and a 2/6 systolic ejection murmur heard best at the parasternal border. The GI exam showed a tender liver. Neurologic: Exam showed orientation x 4 and a 5/5 muscle strength.

George Klein: #050740 - HPI - (1/2 way down)ten block. Is this hyphenated? or is written as 10 block?

George Klein again - 3rd sentence from bottom: The prior hospitalization had a negative _____ gram
