CARDIAC CATHETERIZATION SUMMARY: 1. Hemodynamics: The right atrial pressure mean was 11. Right ventricular pressure was 35/5 with a right ventricular end diastolic of 12. Pulmonary artery pressure was 31/17 with a mean of 22. Pulmonary capillary wedge pressure of 18 mean. Left ventricular pressure was 110/8 with left ventricular end diastolic pressure of 19. Ascending aortic pressure was 115/70 with a mean of 86. Post angiography pulmonary artery wedge pressure rose to an A Wave of 22, a V Wave of 26 a mean of 20. The left ventricular pressure was 117/7 with left ventricular end diastolic pressure of 18 and the ascending aortic pressure was 117/70 with a mean of 80. With a heart rate of 58 the cardiac output and thermodilution was 6.1, index 3.4. Stroke volume 105, stroke volume index 58. AV OČ difference 4.2, stroke index 53. Pulmonary vascular resistance 52. Systemic vascular resistance 83. Quantitative Ventriculography the end diastolic volume is 165, end diastolic volume index 92. The end systolic volume is 62, end systolic volume index 34. The angiographic stroke volume was 103, angiographic stroke volume index . Ejection fraction 0.62 no mitral regurgitation. The RR interval is 102 msec. Politative Ventriculography there is mild anterolateral hyperkinesis. Otherwise the left ventricle appears to contract in a normal coordinated fashion. There is no evidence of intracardiac thrombus. There is no mitral regurgitation. Coronary Anatomy: The right coronary artery demonstrates multiple high-grade lesions, the first one being approximately 90% diameter stenosis. The approximately 1 cm distal to that is 70% mid-right coronary artery lesion, approximately 70% diameter stenosis. Then distal circumflex has a lesion that measure at approximately 70% also. There is also evidence of right-to-left collaterals on angiography. The left coronary system appears normal. There is 100% inclusion of the proximal left anterior descending with late filling of that vessel in its first diagonal. Circumflex coronary artery demonstrates luminal irregularities. However, there is no obvious high-grade lesion. There is a 60% stenosis of the 2nd obtuse marginal and the 1st obtuse marginal is a small vessel.
