I only have a couple of questions this time. It is on Lesson 35 Quiz #23. On Sonya Wells #040409 Dr. Jones says:

IMPRESSION
1. Thyrotoxicosis.
2. Congestive heart failure with atrial flutter.

It may be that the patients’ cardiac symptoms are secondary to her thyroid toxicosis since she has no history of prior heart disease or cardiac symptoms. As her thyrotoxicosis resolves, we want to monitor her digoxin closely as she will be metabolizing it quite rapidly until she is no longer toxic.

PLAN
1. Begin digoxin 1 quarter mg p.o. q.d.
2. Lasix 20 mg p.o. q.d.
3. Tapazole 10 mg p.o. b.i.d.


***The book said to be sure to type headings in the correct order/ Use standard headings for the consultation. Does this mean it should be ASSESSMENT and RECOMMENDATIONS instead or IMPRESSION and PLAN? Also on IMPRESSION/ASSESSMENT should I move the part that is not numbered up to go with #2?

Last question, on the PLAN/RECOMMENDATIONS should it be 1 quarter, 0.25, or 1/4mg p.o. q.d.?

I was told that we were to type in the order dictated in Course 5, with the exception of Operative Reports, which should follow the standard order.

Also, be sure to type out "daily". Somewhere along the way we were told that q. d. is a dangerous abbreviation and should be avoided. I couldn't tell you where or when, just recall that was the rule.
