Hi, can anyone please help me? I have a lot of questions about quiz 38, some might be silly, but i just really need to get a good grade on this test, im due in a couple of weeks, and i really want to finish this before my new baby is born, i would appreciate any help. here are a few on christine lane.
for the heading at the very top, use Weston Medical Center 12383 Lewis Street, Garden Grove, CA 99900 right?
type in no specific order, i mean, type as the dr dictates. correct?
1st paragraph: Oxytocin Challenge Test, the O C and T should be capitalize?
2nd paragraph: is it 42 week's of gestation.. or 42 weeks' ? Glucose Tolerance Test, the G, T and T, should be capitalize?
The patient underwent cesarean section with an estimated blood loss of: 600 cc or mL? or estimated blood loss of 600 cc or mL?
FOLLOW-UP: Return in four weeks to a Family Planning Clinic... is this sentence correct?
and should i make a second page just for the initials, and dates, and lines? i dont have any space on the 1st pg, or how should i do it???

3nd paragraph Having completed this, Dr. Snow was exposing the saphenous vein distally ??? just above the ankle using a sharp incision ... This was used to mobilize the saphenous vein from the ankle up to the proximal ??? region.
4th paragraph Attention was then placed, with self-retaining tractors?? or retractors, im sure its tractors.
whenever hse dictates tib, we whould type tibia, right? at the end of this paragraph, ... ,but the artery was dissected free using sharp dissection placed on vesolups?? ????? and appeared soft.
5th paragraph 7,500 units of Heparin.. The saphenous vein proximally was taken at the bulb by placing a right-ankle 0r right-angle clamp and then transecting it. The proximal rib-net??... The back bleeding was controlled by placing a small Bull-Dog? clamp?? The vein was insoected and found to habe no valves in the bulb or ball junction? i think its bulb. The SFA was then inclamped?? proximally and distally. A side-to-end anastomosis of the SFA.. is this correct?
6th paragraph having completed that, it was backbled? from the distal SFA...
7th paragraph At that time, the in site? in situ? valvulotomies or valvulotomes? The ??? valves in several areas. This was followed then by #3 which after returning the #3 there was ??? bleeding seen in the distal SFA.
8th paragraph With some tension on the vessel ?? and arteriotomy was performed.... Prior to completion of the anastomosis the internal occluder was removed with a lot of good back-bleeding?? The graft was ??? and the anastomosis completed with slight pulsations present at that time.
9th paragraph The endoscope? or angioscope? was introduced... There were no ?? valves seen at this time.
11th paragraph The prvious branches were clipped and ligated with 4-0 silk. is this sentence correct? the foot appeared to have increased ?? than the preoperative cyanosis that had been noted. Staples were placed to the skin, Sulfa and dried sterile dressing and Kerlix were placed. is this right? the patient was then awakened from general anesthetic and taken to the Surgical Intensive Care Unit for further postoperative care in satisfactory condition.

history of present illness: This is a 24-year-old black male with Marfan syndrome, diagnosed 11-years-ago???... Excertion will almost always bring it out, although it also occurs at rest and with anxiety. is this right?
PAST HISTORY: ... Operations: Exploratory laparotomy two years prior to the admission... Medications: Diltiazem 30 mg t.i.d., Inderal 20 mg b.i.d., ??? motrin? Allergies: THE PATIENT IS ALLERGIC TO PENICILLIN WITH HISTORY OF RASH.
Is social history all capitasls and its own paragraph? is family history all capitals and its own paragraph too?
PHYSICAL EXAMINATION
