Anybody still on Quiz 30? Need help! 1.What kind of consultaion report is Susanna Playa? 2.AV nicking and narrowing, ________,__________,_________ or ___________. I have alot of blanks on this one...he is HARD to understand. Any help would be graetly appreciated!!! Thanx!

GENERAL SURGERY CONSULTATION REPORT
Funduscopic examination normal with no evidence of vessel narrowing, AV nicking, capillary aneurysm, hemorrhages or exudates.
Don't worry too much, this isn't a graded quiz.

Hello to all the experts out there. This forum has helped me in my course so far and thank you to all who have assisted me with my questions. So here's another request for help!
Mail In Quiz 31 - Reports 12.
Sergio Cortez 1. If the name of the hospital is not dictated, do I have to type in a name at the top of the page. I am currently doing a path report and no hospital has been dictated.
2. Also, the dictated date collected is June 7, - can I insert 2011, since June hasn't arrived yet in 2012?
Lee Sung 1. GROSS EXAMINATION was dictated instead of GROSS DESCRIPTION. Which is correct?
Autopsy Report John Ford 1. Under ANATOMICAL SUMMARY Respiratory System. The dictation states - The lung parenchyma is crepitant (it sounds like Rice Crispies. It is air under the skin and the tissue) and hyperaerated and shows congestion of the posterior portions. The cut surfaces.....etc.
Do I type the parenthesis and what follows until closed? I personally think this is something being stated and not part of the report. Any comments?
2. Urinary System: The cortical surfaces of the kidneys are smooth. There are no scars. The cut surfaces show red cortices and good cortical medullary demarcation.
Not sure if I heard this correctly. Please advise.
LABORATORY DATA Sodium 135, potassium 4.9, chloride 92, CO 220, BUN 33, creatinine 1.1, sugar 210, ALT 37, AST 66, alkaline phosphatase 91, PT 96% of normal, LDH 378, CK 301, total bilirubin 0.4, direct bilirubin 0.1, total protein 7.8. Urinalysis was normal. Blood counts showed hemoglobin of 16, hematocrit of 45, 44% segmented PMNs, 41% banded PMNs, 7% lymphocytes, 8% monocytes, the white count was 9,700. The EKG showed right atrial enlargement.
Not sure if I put commas and stops in the correct places. Please help.
I would like to get this out by Friday if possible.

Are you with AHP? My Quiz 31 is Reports 16 rather than 12, and I do not have a S. Cortez, L. Sung or J. Ford in 31.
