Hello, I am new on here. I am on Quiz 24 lesson 11. The History and Physical Examination for Miles Rogers has been going alright except for a couple of parts. I want to make sure that I am hearing the part in Physical Examination where it says Trachea midline. TMS clear. Pharynx not examined.

PLEASE HELP!! I have been confident on parts of reports before and those are usually the ones that get torn apart. Thank you!

Hi all. I am new to this site. I am taking the At-Home-Professions course and have been since February 2010. I have recently started up after a long break and just received my grade back from Quiz 23 and it was terrible. I am very discouraged having an ASB in Medical Transcription already, but going on to take this course to get further training.

I was wondering if anyone could help me with my finished Quiz 24 and 25 as I am reluctant to send them in and have been aimlessly mauling over them. Another pair of eyes would be much appreciated.

For Quiz 24 Brandon Lang, on the Physical Examination part. Where should you add in the Weight and Mouth headings? Also, should they be capitalized?

Then for Miles Rogers under Plan....it says...

NPO lateral neck film to rule out subglottic edema. 30% oxygen mist tent. Racemic epinephrine 2cc (1:16)(not sure about that in parenthesis). Decadron 5 mg. Tylenol p.r.n. for fever. Intubation percussions (???)
until radiographic evidence of subglottic edema is excluded.

I am still relistening to Quiz 25.

Weight and Mouth are subheadings and they are initial caps. This is how it s/b:
HEENT: Reservoir firm. PERRL. EOMs full. Ears: Tympanic membranes clear. Nose: Clear. Mouth: Clear. Throat: Clear.

PLAN
NPO. Lateral neck film to rule out subglottic edema. Thirty percent oxygen mist tent, racemic epinephrine 2 cc (1:16). Decadron 5 mg. Tylenol p.r.n. for fever. Intubation precautions until radiographic evidence subglottic edema is excluded.

I know better than to start a sentence with a number (30%)!! I can't believe I did that one. It did help and I thank you kindly. I am still amazed at the score I received, but after going through some of the threads posted on here I guess I have been blessed with an encounter of the "hard hit" of the "bad" teacher who grades very harshly. I have never had her grade my quizzes until this one. Hopefully, I never will again, yet I hope to learn from this experience as well.

I thank you for your time in helping me. It is much appreciated to have this site to turn to now when all hope seems to be lost.

Also for Brandon Lang....

ABDOMEN: Soft normal bowel sounds vesicostomy. No hepatosplenomegaly.

Would I add commas to the first sentence? My first thought is to put one after soft and then after sounds but always seem to have trouble.

I have trouble with commas too. Have this quiz back & graded & this is what I have & it is not marked wrong.

I am slapping myself in the forehead. I had just that before I listened to the track again. Now, after doing bad on my last Quiz I am just second guessing everything....I think in the long run all I am doing is making things worse for myself....

Do NOT slap yourself in the forehead, that only gives you a headache.
You may be right, about making it worse for yourself in the long run. Many times when I feel that confused, I walk away & do something different & then re-listen and will get a clearer "listen" on the dictation. Just what "sometimes" works for me.
Just do not be too hard on yourself, that is why this site is a great help.
If you want to get faster results from your posts, just post on the top topic to the right of the page, Popular Topics & go the the largest number listed to the right of that, I think that is where we posted B4. Seems people see things there sooner. Just a thought. Have a good nite & good luck.
