Can someone please tell me if I have this entire REVIEW OF SYSTEMS set up right or should I add "General:" before - He is 6 feet tall....and also should I have EENT as a subheading and any other suggestions would be SO helpful - thank you!

REVIEW OF SYSTEMS
He is 6 feet tall and weighs 260 pounds. His weight has been stable. No rashes.
EENT: No problems with his eyes, ears, nose, or mouth.
Respiratory: He is positive for smoker, half a pack per day, with a smokers cough.
Cardiovascular: He has a history of hypertension.
Gastrointestinal (GI): No problems.
Genitourinary (GU): No problems.
Musculoskeletal: As stated in the history of present illness. No neurological or hematological disorders.

forgot to mention that this is a HISTORY AND PHYSICAL EXAMINATION

This depends on the rules of your client/school. Do they want the subheadings justified left or in paragraph form? Do they want you to input subheadings when not dictated? Also, since there is a section on the patient's weight, it seems more like a physical examination to me.
