Can anyone help me figure out what he is saying? I've listened to it over and over and can not figure it out. Any help would be appreciated. Thanks

One week prior to admission, the patient saw a television program suggesting that Zoloft might have long-term adverse effects. She stopped the medication abruptly. Over the next week she became increasingly nervous, emotionally labile, and suffered from insomnia. Two days prior to admission, she fell in the bathroom. She did not recall specifically how she had landed. Since that time, however, she complained of lower abdominal discomfort. She also noted ecchymoses and "ontromblelychis".

She also noted ecchymoses around her umbilicus. Hope that helps!

Hello,
I am presently working on this discharge summary. I would greatly appreciate any tips:) It IS making me crazier.

3. Status post thyroidectomy for ____("fortune") carcinoma.

She had undergone a total thyroidectomy for _____("??") carcinoma.

I have been researching what this type of carcinoma is but I am coming up short.

I am taking the Allied course also, and the preparatory research portion at the end of the chapter should help, it did me. I always find this section after each chapter most helpful, having helped to point me in the right direction countless times. I am still having a hard time "developing an ear" with regard to some dictators, does anyone have any tips that can help?

Yay, there it was! Shame on me for not looking through that portion better. Regarding the developing an ear, you are not alone. I am looking for a better headset solution, maybe with a volume control attached. Too much noise around me at home. Have you finished the course yet?

I haven't finished yet, I am on chapter 16. I was doing fine until recently, when I appear to have hit the wall and have taken a nosedive, hopefully a temporary one.

My husband is in the electronics/recording industry and has given me a pair of professional headphones which has helped tremendously over the cheapy pair we received in the school package. The only problem is if the source tape is scratchy or the dictator doesn't enunciate clearly, I am just lost.

There was one chapter where the dictator (I know she was tired) sounded like she had been drinking, she was slurring her words and yawning. Ah, a preview of what's to come.

Hopefully it gets easier on the job, becoming more familiar with the subject matter and context in which the reports are dictated. Anyone have any suggestions of a good place for us fresh-out-of-school newbies to look for a job?
