When one of the docs dictates, "I examined the patient in a gown," I have this vision of HIM elegantly sweeping into the exam room in a pink flowing taffeta and tulle formal evening dress!

Reminds of other mis-charted items, often which I have thought were likely typo's (swiped from a corner of a web and around longer than me)
