So I spent about 5 min. scanning my pt list today for David Berth..... Frustrated, I gave up and listened again. This time I heard Date of Birth.. go figure!

Good one, Racheal!!

One of my favorites was when one of my docs dictated "ADLA 6 a day." I researched and listened and couldn't get it. Finally went away for a couple of hours, came back, and it was very clear: 80 of Lasix a day!

That is a good hint for newbies--come back to it later, because it might sound different then!

The docs don't realize sometimes it's all in the phrasing! I'm sure if your doc had paused ever so slightly between words and not run dateofbirth together you would not have had to waste all that time!

LOL! I typed several letters to a Dr. Ewes and kept flagging them because I could not find Dr. Ewes anywhere. Well, it was Dr. Hughes. Boy did I feel like a big DUH!

Okay, here is one I just had - "he thought it was actinic keratosis and burned bath." What and the heck is burned bath? I listened about 10 x and started to google it, and then it hit me - "burned that." I could probably post these kinds of things all day every day smile

I just came across a funny that someone else (in the pt's EMR) had put...

patient (with ED) "complains of rectal dysfunction which interferes with his sex life" - should have been erectile... though if the first half were the problem, the latter wouldn't be hahaha

one I'm remember from a few years ago that was quite funny went along the lines of: "Patient comes in complaint of bowel problems and notes pain with dedication." (I think a typo + spell checker was responsible for that one).

An error of mine that ended up printed off and passed around the office: "Patient is in with swallowing difficulty, noting he keeps getting foot stuck in his mouth." Goodness that typo was a good laugh!

Oh these are too funny!! I have one myself. The patient was to take Tenormin 25 mg daily. Well, I put the patient is to take To Norman (as in a town). Boy was my face red!!
